3. How to Use the Provider Directory to Find and Add Referral Recipients

What is the Provider Directory?

The Provider Directory is a national list of care facilities: Skilled Nursing, Home Health, Hospice, Dialysis, Long-Term Acute Care (LTAC), and Hospitals. A provider is validated once, and that information is then used by everyone across exacare who interacts with them — you don't need to enter or maintain your own version of it.

There are two separate things you can do with it, and it's important to know which one you're doing:

  1. Configure your own facility: This is about being found. You're publishing your facility's contact info into the shared directory so other organizations can find you and send you referrals.

  2. Find and configure a facility as a recipient: This is about reaching out. You're searching the shared directory for another facility and saving the details you need so you can send them a referral.

You don't need to do both, and doing one doesn't require the other. The sections below cover each one separately.


Configure Your Own Facility

Purpose: For other facilities/agencies/locations to find and reach you.

What this does

This publishes your facility's contact information into the shared, national Provider Directory. Once set, any organization - not just facilities you already work with - can find your facility and send you a referral with the right contact already attached. You are setting up how others reach you, not searching for anyone else.

Before you start

You need Admin Hub access.

Steps

  1. Go to Admin Hub → Facilities.

  2. Open your facility.

  3. Scroll to the Provider Directory contacts section.

  4. Set two contacts:

    • Admissions & Intake: The contact for referrals sent to your facility.

    • Discharge & Transition: The contact for referrals sent from your facility.

  5. Save.

What happens next

Your facility becomes discoverable in the Provider Directory using the contacts you set. Any exacare user searching for your facility as a recipient will see this information automatically - you don't need to tell them what it is separately.


Find and Configure a Facility From the Provider Directory

Purpose: To send other facilities / agencies / locations a referral.

What this does

This is for when you want to send an outbound referral to a facility that isn't already set up as a recipient in your account. You search the shared Provider Directory for that facility and save the contact details you'll use to reach them. You are configuring your list of who you can send to - this has no effect on how other exacare users see your own facility.

Before you start

You need standard access to the Outbound tab.

Steps

  1. Click the Outbound tab in the left-hand navigation.

  2. Click Configure recipients.

  3. Search the Provider Directory for the facility you want.

  4. Fill in the contact details.

  5. Click Save Recipient.

What happens next

The facility now shows up in your recipient list the next time you send an outbound referral. If that facility also uses exacare, your Provider Directory identity travels with the referral, so they see accurate source information on their end automatically.


FAQs

How does this relate to the exacare "Sources" table I already use?

When you use the Provider Directory to send a referral in exacare, it automatically populates Sources with accurate information for the recipient.

Do I need to configure both my facility and my recipients?

No — they're independent, as covered in Parts 1 and 2 above. Configuring your own facility (Part 1) lets others find and reach you. Configuring a recipient (Part 2) lets you send to them. You can do either without doing the other.

What if the facility I'm looking for doesn't show up in the search?

Double-check the spelling or try searching by a different identifier. If the facility genuinely isn't in the national directory yet, you can still add it as a recipient with the contact details you have - contact exacare Support if you'd like it added more broadly.
